United Way Donor Choice
Agency Eligibility Application
To become a member of United Way of Southeastern Pennsylvania’s
Donor Choice Program, agencies are required to complete an Agency Eligibility
Application. Generally, agencies are mailed an application as the result
of a donor designation. Agencies may also apply directly by submitting a
completed application and ALL required documents with a nonrefundable application
fee of $100; payable to United Way of Southeastern Pennsylvania by check
or money order. The purpose of the application fee is to cover the cost of
the application research and review process and does not guarantee acceptance
into the program.
For an agency to be eligible, each element of the core criteria must be met.
An agency must:
- have a federal tax exempt/tax deductible status of 501(c)(3)
- provide services
to the residents of Pennsylvania, New Jersey or Delaware
- exist primarily
to provide or support voluntary human health or social services
- comply with
the USA Patriot Act
Examples of ineligible fields of service:
- Religious Institutions
- Libraries
- Animal Welfare
- Disarmament, Nuclear Weapons, Peace Groups
- Environmental Organizations
- Government Agency Service
- Economic Development
- Cultural Organizations (museums, orchestras, theaters,
ballets)
- Schools, Colleges, Alumni Funds (Public or Private)
Organizations which fall within an ineligible category may have a program
or service that separately meets our eligibility criteria. The program/service
must make up a minimum percentage of the organization’s overall budget
and appear as a lined item on their financial report. Please complete and return
the Significant & Distinct
Program Worksheet with your application if your
agency falls within the ineligible category, but provides an eligible service
or program.
The application and samples of the required documents are below. Please answer
all questions on the application - the more you tell us about the purpose and
activities of your organization, the better we will be able to determine whether
you are eligible to participate in the program.
