Project Consultant for Community Networks of Care
Effective January 5, 2012: United Way of Southeastern Pennsylvania announces a Request for Proposals for a Project Consultant. The Project Consultant will work with the Healthy Aging priority area through “Community Networks of Care” (CNC), an initiative funded through United Way of Southeastern Pennsylvaniaand supported in it’s research phase by the Pennsylvania Department of Aging. Community Networks of Care is a multi-year initiative that will implement and test at least one, and up to three, new models for coordination of home and community based care for older adults. It is intended that at least one model will be financially sustainable, with replication possibilities across the region.
This Project Consultant position is a “Financial Sustainability Analyst”. The work of the Financial Sustainability Analyst involves investigating current funding streams and activities eligible for reimbursements under the Federal and state reimbursement systems. The information gathered would then inform the development of CNC models. This position is intended to help the CNC Governance Team better understand what might qualify for Federal Financial Participation (FFP). This is intended as a 10 month contract, with a report due after 30 days, and additional consulting as needed to provide technical assistance to the first Community Network of Care model implemented.
